EMF MEMBERSHIP APPLICATION FORM

Yes, I want to apply for EMF membership. I hereby subscribe to become a Member of the European Multimedia Forum and undertake to abide by the European Multimedia Forum Rules.

MEMBERSHIP CRITERIA

We are a corporation and undertake to pay an annual membership fee of 2000 EUR - annual revenues > 5 million EUR.
We are a corporation and undertake to pay an annual membership fee of 1000 EUR - annual revenues between 5 million EUR and 2 million EUR.*
We are a corporation and undertake to pay an annual membership fee of 500 EUR - annual revenues between 2 million EUR and 1 million EUR.*
We are a corporation and undertake to pay an annual membership fee of 250 EUR - annual revenues between 1 million EUR and 250.000 EUR.*
We are a corporation and undertake to pay an annual membership fee of 150 EUR - annual revenues < 250.000 EUR.*
We are a corporation / institution and seek to become an Associate Member (NO FEE - NO RIGHTS).
We are a membership association, please bill us 1000 EUR. We understand that all our members will thus automatically acquire member status of EMF.
I am an individual and would like to apply to become an Advisor to the EMF.**
* Please forward a letter certifying that your annual revenues are below 250.000, 1, 2 or 5 million EUR.
** After review of your qualifications, you will be informed if your application has been accepted.
